Compassionate Palliative and End of Life Care in Pakenham, Officer & Narre Warren
At RVP Care Services, we believe every person deserves to live and be supported with dignity, respect, and peace at every stage of life. For individuals with life limiting conditions, our compassionate palliative and end of life care focuses first and foremost on comfort, connection, and quality of life.
Whether in your own home, a Supported Independent Living (SIL) house, or the community, our NDIS registered support workers are trained to provide gentle, respectful, and person centred care guided by your wishes, your values, and your plan.
What Is Palliative and End of Life Support?
Palliative care is not about giving up. It’s about living well with comfort, meaning, and support. When facing a serious or progressive illness. End of life care is the natural extension of this support, ensuring safety, symptom relief, and emotional wellbeing in a person’s final days or weeks.
For NDIS participants, this type of care may be part of your plan when goals shift toward comfort, stability, and being surrounded by loved ones and trusted carers.
How We Provide Compassionate Support
At RVP, our approach is always individualised. No two people are the same, and neither are their needs, hopes, or fears. Here’s how we help:
🔹 Person Centred Care Plans
We work closely with you, your family, GP, palliative care team, and NDIS support coordinator to create a plan that reflects your priorities, whether that’s staying at home in Pakenham, enjoying quiet moments in the garden, listening to favourite music, or simply having a familiar face nearby.
🔹 Symptom Comfort & Daily Living Support
Our trained support workers assist with:
- Gentle personal care (bathing, dressing, positioning)
- Mobility and comfort supports (pressure relief, repositioning)
- Medication prompting (as permitted under NDIS guidelines)
- Hydration and nutrition support, including enteral feeding (PEG/NG) where required and in line with clinical advice
🔹 Emotional & Social Connection
We know presence matters. A quiet conversation. Holding a hand. Sharing a story. Playing a cherished song. Our team offers companionship without pressure, honouring silence as much as speech.
🔹 Family and Carer Support
Caring for someone at the end of life can be emotionally and physically demanding. We support families and informal carers too, offering respite, practical help, and a listening ear, so you can focus on what matters most: being together.
🔹 Culturally Safe & Inclusive Care
We respect your culture, faith, language, and identity. Our team includes support workers experienced in working with people from diverse backgrounds, including those with psychosocial disability, acquired brain injury, or complex health needs.
Where We Provide This Support
- In home care: across Pakenham, Officer, and Narre Warren
- SIL houses: where we create calm, homely environments with 24/7 trained support
- Community settings: helping you stay connected to what brings joy, even in later stages
All our team members complete ongoing training, including in epilepsy first aid, emergency response, complex care, and trauma informed support, because safety, skill, and kindness go hand in hand.
A Note on Choice and Control
Under the NDIS, you have the right to choose your supports, including when goals change. Many participants include palliative and end of life care under Core Supports (Assistance with Daily Life) or Capacity Building (Improved Daily Living or Improved Relationships), depending on individual goals. Our support coordinators, including those at our sister organisation, Ideal Care Services, an NDIS registered plan management and support coordination provider can help you review your plan, connect with medical teams, and ensure your care aligns with your values.
You Are Not Alone
Facing life limiting illness is difficult, but it doesn’t have to be faced alone. At RVP Care Services, we walk alongside you with empathy, professionalism, and deep respect, helping you feel safe, heard, and valued, right up to the very end.
